IN PERSON - Strategic Conversations Series: Connecting Community Members to Resources in the Inland Southern Region
Conversation Theme: People don’t live in hospitals, they live in communities. As such, we recognize that health goes beyond medical care to include social care. We must then work collaboratively to ensure that community members are receiving the resources that they need. Join us to hear how free-of-cost access to the Unite California Network for all community-based organizations is leading to changing the traditional care delivery model to one that holds providers accountable and goes beyond the referral.
Presenters: Maria Pelaez is a Community Engagement Manager at Unite Us, based and serving Riverside County. Maria has experience in creating and facilitating non-profit programs that promote access to local resources to underserved populations throughout Southern California. Through her commitment to serving her community, Maria continues to advocate for all community members to have equal access to resources through Unite Us. She engages with community-based organizations throughout Riverside County to know more about their needs and invites them to be part of a larger network that gives them access to both local and state-wide resources. Maria’s pledge to work towards the welfare of her community goes beyond her role in Unite Us as she is an avid volunteer of community organizing and policy advocacy groups.
Jasmine Medina is a Community Engagement Manager at Unite Us, located in and proudly serving San Bernardino County. Jasmine has a background in afterschool-based and organization health education, peer mentoring, and community outreach to underserved populations. Prior to joining Unite Us, she created and facilitated wellness programs for at-risk youth and their families living in affordable housing communities, with the goal of teaching them to become self-advocates for their health and the health of their communities. She continues to support these communities by building networks of changemakers, supporting health equity, and making services more accessible in the Inland Southern Region.

Please note that MapQuest does not give the correct directions to this site ... Interstate 15, Exit "D Street/Highway 18 East" and travel East. Travel two miles through Victorville and up/down a short hill into Apple Valley. The first traffic light in Apple Valley will be Apple Valley Road. The facility is at the Northwest (left) side of the intersection of Apple Valley Road and Hwy. 18, behind the gas station, in the center of the strip mall. Parking is in the lot in front of the building. Please do not park in short-term spots or you will be towed. There are two Highway 18 exits. Make sure you take the one that says "D Street."
